Qualifications
- Candidates must hold a current state license as a Physician Assistant, with a graduate nursing degree from accredited school or Physician Assistant program
- Experience in direct patient care, particularly in the homecare setting, is highly valued
- Strong clinical acumen, excellent communication skills, and a passion for innovative wound treatment are essential
Responsibilities
- In this role, you will assess, diagnose, and manage patient’s wound care directly in their homes

- You’ll collaborate with a multidisciplinary team, leverage advanced clinical tools, and help drive our mission to improve wound care outcomes
- You will conduct comprehensive patient evaluations, develop individualized treatment plans, and utilize advanced wound treatment technology
- The position requires a proactive approach to patient education, follow-up, and coordination with our robust network of clinical resources

- Assess patient wounds and develop a comprehensive treatment plan
- Debridement of wounds by sharp, enzymatic or autolytic methods
- Application of topical wound care and associated wound management products
- Recommendation of associated services to improve wound healing outcomes
- This includes, but is not limited to, labs, radiologic tests, vascular tests, referrals, home health care, Negative Pressure Wound Therapy device, and orthotics
- Interpretation of diagnostic tests and recommendation of appropriate follow up
- Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician
- Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines
- Patient education and management
- Education of the Primary Care Physician (PCP), Case Manager (CM), or referring Physician on current wound management practices
- Provide patient updates to PCP, CM, and Physicians
- Communicate in a clear and understandable manner using appropriate verbal and non-verbal styles
- Actively participate in departmental activities to improve training methods and achieve company goals
- Assist with one-on-one training and coaching sessions as assigned
- Attend required meetings and participate as a committee member as requested
- Assist with special projects as needed
